Are you healthy, and height/weight appropriate? (32 BMI or less)
Are you drug free, and alcohol free?
Are you a non-smoker?
Are you free of any felony convictions?
Do you have reliable transportation and live in Illinois?
Are you a U.S. citizen or have a green card?
